Hospital District Operations and Obligations

Hopkins County Hospital District (HCHD) was established in 1961 by the 57th Texas
Legislature as a governmental entity created to ensure access to healthcare services
within Hopkins County. Under Texas law, hospital districts are formed to provide and
support healthcare services, with particular emphasis on indigent care. Currently, Texas
has 142 hospital districts, covering approximately 56% of the state’s 254 counties.

Governance Structure

Hopkins County Hospital District is a governmental entity; however, it does not govern
Christus Mother Frances Hospital – Sulphur Springs. The hospital is a private nonprofit
facility operated by Christus Health. While the District and Christus Health operate under
contractual agreements, governance authority for the hospital rests solely with Christus
Health. The District does not manage hospital administration, clinical operations, or
strategic direction of the hospital or its affiliated clinics. However, Christus Health and
the District have aligned strategic goals and objectives for our community’s healthcare.

Service Lines and Direct Operations

The only service line directly operated by Hopkins County Hospital District is Hopkins
County EMS, including the Emergency Medical Communications Center for the
immediate region. As owner-operator of Hopkins County EMS, the District maintains
responsibility for emergency medical services that are active locally, regionally, and at
the state level. The District does not directly operate hospital and clinic operations. All
inpatient, outpatient, and clinical services provided at Christus Mother Frances Hospital –
Sulphur Springs are managed by Christus Health under the terms of the contractual
arrangement with the District.

Relationship with Christus Health

Hospital and clinic operations are structured through an extensive agreement between
Hopkins County Hospital District and Christus Health. Under this arrangement, the
District owns the majority of the hospital campus property and buildings. Christus Health
leases the facilities from the District, and the agreement functions similarly to a
commercial lease structure. Through this lease and operational agreement, Christus
Health operates the hospital as a private nonprofit entity. While the District retains
ownership of the physical assets, it does not exercise operational control over hospital or
clinic services.

Indigent Care Responsibility

Despite not operating the hospital directly, under its enabling legislation and the Texas
Constitution, Hopkins County Hospital District maintains responsibility for providing
medical and hospital care for the District’s indigent residents. The District fulfills its legal
obligations through funding structures and contractual agreements with Christus Health
to ensure that qualifying residents have access to necessary healthcare services. This
framework allows the District to meet legally mandated indigent care obligations while
leveraging the operational capacity of a private nonprofit healthcare system.
Quality Incentive Payment Program (QIPP) Participation
Hopkins County Hospital District participates in a cooperative Quality Incentive Payment
Program (QIPP) with the Texas Health and Human Services Commission (HHSC).
Through this initiative, the District partners with nursing facilities to improve quality
metrics and resident outcomes. The program utilizes performance-based payment
incentives to promote higher standards of care while leveraging state and federal funding
mechanisms. Participation in QIPP represents a strategic financial and quality
improvement initiative that is distinct from direct hospital operations.

Summary

Hopkins County Hospital District is a legislatively established governmental entity with
statutory obligations centered on healthcare access and indigent care funding. Although it
owns the hospital campus property, the District does not operate or govern Christus
Mother Frances Hospital – Sulphur Springs; instead, hospital and clinic services are
managed by Christus Health under a structured lease agreement. The District’s only
directly operated service line is Hopkins County EMS and the Emergency Medical
Communications Center. Through its contractual relationship with Christus Health and
participation in the Quality Incentive Payment Program, HCHD fulfills its legal
responsibilities, supports regional healthcare access, and strategically enhances healthcare
quality and funding stability within the community.
